How We Can Help You Through The Steps To Aged Care
Step 1 - Aged Care Assessment (ACAT)
The first step is to obtain an assessment from your local Aged Care Assessment Team (ACAT). This can be arranged through your loved one’s doctor.
Step 2 - 1:1 Consultation
Once the assessment has been completed, our consultant will meet with you and your loved one to determine your needs based on the assessment.
Step 3 - Complete Centrelink Application
Once we have assessed your needs, we will help you through the Centrelink application process. This is a vital step and needs to be completed accurately as it will determine how much the resident is required to contribute towards his or her care. We may need a second meeting to ensure that all necessary information is provided to Centrelink.
Step 4 - Identify An Aged Care Facility
Once Centrelink’s assessment has been received, we will help you to find an aged care facility that best suits the needs and requirements of your loved one. Because we are totally independent of the nursing home providers’ we can provide you with an objective assessment of which facility is likely to be the most appropriate. We will always try to provide at least three options. We are also happy to attend on-site inspections of the recommended facilities with you and will discuss your options with you in light of our joint observations.
Step 5 - Advice And Assistance
Once you have chosen a facility, we will negotiate the cost of entry with the provider and help you understand the legal implications of the resident agreement that you will enter into with the aged care facility. Once this is done, we will provide you with a written summary of the obligations created by the agreement.
Step 6 - Ongoing Advocacy
Once your loved one is in care, we don’t see our role as being completed as we believe that we should, and will, be available to advocate on behalf of our clients while they remain in care.
